P.O. Box 35070, Louisville, KY 40232-5070. Field Supervisor: Ms. Cindy Horn, L.C.S.W., 502/629-8889. Student Intern: Performed psychosocial assessments on clients entering the program. Participated in multi-disciplined treatment planning. Organized and facilitated a weekly group on communication and social skills. Co-facilitate daily community meetings and group treatment. Provided case management and located community resources for the clients assigned to me. Charting. Summer - Fall 1993.

Home of the Innocents's Transitional Home for Young Families,

485 E. Gray St., Louisville, KY 40202. Field Supervisor: Mrs. Karen Laidlaw, M.S.S.W., 502/561-6600. Student Intern: Provided informal counseling to parenting teens on issues such as parenting, relationships, handling authority, educational and vocational issues, personal boundaries, problem-solving, and independent living issues. Co-led weekly Rap Group. Researched, designed, and provided a staff training on incest. Participated in staff meetings, in-services, and outside training events (i.e., "Serving Africian-American Children with Severe Emotional Disabilities and their Families"). Compiled an Independent Living Manual of community resources and other relevant information for young mothers. Participated in quarterly case conferences, hospital visits, weekly resident meetings, Mayor's Task Force on Teen Pregnancy, and more. I researched and compiled "Recommendations for Home of the Innocents Regarding Psychiatric Therapeutic Interventions". Summer - Fall 1992.

* 1989-1992 Member of the Affirmative Action Committee at Colgate-Palmolive Company. This group meets monthly.

Work Experience not related to Clinical Social Work

Colgate-Palmolive Company, Jeffersonville, IN 47130, 812/283-6611.

Position: Chemist. Duties include normal wet lab tests for process control and raw materials. 1990 to present.

Position: Q. A. Inspector. Duties included inspection of finished products for general appearance, weights, code dates, etc. When quality problems occurred, I helped to locate the source of the problem(s). January 1982 to 1990.

Borden Chemical Company, Louisville, KY 40216.

Position: Research and Development Laboratory Technician. Duties included lab tests on phenol-formaldehyde resins and others including hot plate cure, glass plate flow, gel time, viscosity, specific gravity, pH, tensiles, etc. I also had a very active role in the training of new lab technicians assigned to test production for the new phenolic plant when it started up. 1977 to 1979.
